PLEASE NOTE YOU ARE REQUIRED TO PRESENT YOUR OHIP CARD AT EACH VISIT.

All new patients must be referred by a medical doctor, or an Optometrist along with a medical doctor. When booking your appointment your doctor will provide us with appropriate written information to allow us to assess the urgency of  your visit, so that we can schedule your visit appropriately.

You can now e-mail us with new appointment, cancellation, or rebooking requests at booking@eyemd.bz .We will e-mail back an appointment date within 5 working days. Please be aware that appointments are booked on both a priority and first come first serve basis. If your visit is urgent please call us directly, or ask your family doctor to arrange the appointment for you. If your family doctor has asked you to see us please bring a consultation request note with you. Please follow the instructions below:
